125-pound weight loss from October 2022 – July 2023. How much weight would you like to lose? My starting weight was a shameful 325 pounds. I was on a path of heart disease and diabetes if I didn’t make a change. When you can’t keep up with your 2 year old or your 6 year old things need to change to be there for them when they need you the most. My journey started with Tirzepatide, I was on this weekly until July when I switched to Semaglutide. Tom my LPN in the clinic started his journey in December of 2022 and by July 2023 he had also lost 120lbs, along with lowering his Hemoglobin A1c from 6.8 to 5.2.

Semaglutide
Semaglutide is a GLP-1 (glucagon-like-peptide-1) medication. This class of medication was first FDA-approved in 2005. Almost 20 years of research has been performed on this class of medication. The class has been refined over the years. In 2017 Semaglutide was first FDA-approved for type II diabetes and in 2021 Semaglutide was FDA approved for weight loss.
Cyanocobalamin is a form of B-12. This is compounded in the injection. This could increase energy levels if you are on a calorie or carb-restrictive diet plan.
The dosage ranges from 0.25mg to 2.5mg at the maximum range for continued maintenance. Medication is given subcutaneously in either your lower abdomen, the back of your upper arm, buttocks, or upper outer thigh. The needle size is similar to an insulin syringe needle, which is smaller than your traditional intramuscular injections (Vaccines, Testosterone, B-12 injections). Many patients find it easy to self-administer, others find it easier to have their partner or friend administer the medication.
The medication slows gastric emptying, which could interfere with the breakdown of other medications. Make sure a full list of your current medications and supplements are given to your ordering provider.
Patients who have a history of medullary thyroid cancer or other thyroid C-cell cancer, a history of pancreatitis, tachyarrhythmias, and are pregnant or breastfeeding should be wary of taking this medication.
The medication has been very effective in the reduction of patients’ weight and A1c. It should be noted the first month, while on Semaglutide, you will be placed on 0.25mg, which is not a therapeutic dose. Many patients start noticing the biggest reduction in weight at month two and beyond. The reason for starting at a low dose is to monitor for reactions to medication and to minimize side effects. After four weeks the medication will be increased in strength.

1. What is Semaglutide?
Semaglutide is a long-acting analog of GLP-1 that was FDA approved in 2021 for the treatment of obesity and diabetes.
2. How does Semaglutide work?
It is injected into the body subcutaneously, usually the stomach or another fatty area and targets receptors in the body related to appetite and hunger.
3. How much weight will I lose if I take it?
This is solely dependent on the patient. Patients who are severely obese or morbidly obese will lose faster than patients who are either overweight or obese. Adding a workout regimen and eating plan will also help in the amount of weight lost.
4. How much of the medicine will I need to take?
It is a progressively increasing dosage of a once-a-week subcutaneous injection. Dosages range from 0.25mg to 2.4mg.
5. Will I still be hungry and what can I eat?
Most patients find smaller servings satisfy their hunger and there are no specific food restrictions. If you want to maximize the effectiveness of the medication then adhering to either a calorie deficit or low carb diet will result in higher weight loss.
6. What are the common side effects?
The most common are GI side effects which include nausea, vomiting, constipation, abdominal pain and diarrhea. These can be easily controlled by adjusting how much you consume and what you consume while taking the medication.
7. What will happen if I stop taking the medication?
The analogy I use with my patients pertains to weight loss surgery. Patient can have a gastric bypass or a gastric sleeve and it can be super effective in the reduction of weight. If a patient does not continue life style adjustments when it comes to food and drink the weight will come back regardless of what you did to lose it. You can either take this medication long term or take it short term to get you back on track to a healthier lifestyle to keep the weight off.
8. How much does the medication cost?
The cost for the medication is dependent on the dosage requirement for the patient. Because the dosage amount increases with usage, the price will also gradually increase.
